If you can't find a form you need here, please come into the Sabbatical office and collect it from the Sports officer, or contact them here.

Event night club guest lists

•  Clubs guest list request form for event nights (e.g. Purple Wednesdays)
  Download and complete this form if you want your club to get priority access to Purple Wednesdays and other participating event nights.

Website application form

  Use this form to have your site created on UPSU.net or to have your existing site linked from UPSU.net - this helps you get more members by raising awareness of your club or society on the web.
